- You are required to complete a Lodge 9 / FOP MedallionDisclosure Form for each medallion purchased.
Anyone displaying F.O.P. medallions or active member stickers who is not authorized to do so, can be charged under the following Illinois State Statute : 720 ILCS 5/17-2.
